This form is meant for students who use extra support during written and digital exams. You and the study advisor have already aligned on what the support is. The examinations office regulates the practical organisation of your assigned facilities
WHAT CAN YOU DO WITH THIS FORM?
- When logged in, the form will collect your UT contact details automatically.
- You can send us requests for extra facilities for up to 10 exams.
- You can send us cancellations for up to 5 exams.
APPLYING FOR EXTRA FACILITIES:
Your study advisor needs to have informed us about your assigned examination facilities. The exam office will check this upon receiving your request.
You don't need to apply in advance for extra time during exams (with or without a dictionary).
There's no need to fill out the extra facilities form for this.
Location: You take the exam in the regular exam room.
Other extra facilities
To use additional facilities, e.g. a laptop, text-to-speech software, or a separate room, you must apply at least 14 days (10 working days) before each exam.

WHAT WILL HAPPEN AFTER YOU SEND US THE REQUEST?
- Someone at the exam office will receive the email and start working on your request or cancellation.
- The exam office has 10 working days to process your request. The duration depends on multiple factors. Cancellations are processed within 3 working days.
- The exam office will check your request for the assigned facilities
- After processing the request the exam office sends you an email.
OTHER IMPORTANT INFORMATION
Extra facilities card:
You will receive an Extra facilities card that shows which extra facilities you are assigned. Bring this card to every exam and put it on the table next to your student card.
